Cera is Europe’s largest provider of digital-first in-home healthcare, completing more than 60,000 home visits every single day. Together with Faculty, they developed a predictive tool that used existing data points recorded by Cera’s carers to identify potential health conditions and hospitalisation risks.

The technology predicted 80% of hospitalisations more than a week in advance, and by introducing rapid and low-cost interventions, Cera was able to avoid 52% of these hospitalisations.

We made this film with carers Kenza and Cristina, who told us about life as a carer and the impact the tool has on their day-to-day work.
